Welcome to Inspire Horizons - discover how we do things differently!
At Inspire Horizons, we're not your typical therapy center. We’re building a united community that brings together experts in Occupational Therapy, Physical Therapy, Speech Therapy, and Applied Behavior Analysis (ABA) to break down barriers to help children (typical ages 2 – 8) with developmental delays and disabilities

Wondering what it's like as an Occupational Therapist at Inspire Horizons?
  • No travel: We don’t do home services, and you’ll work at your assigned location every day.
  • Guaranteed consistent compensation: regardless of a patient cancellation or indirect administrative time.
  • For full-time team members, one hour of paid admin time every day: We don’t believe in back-to-back sessions all day long. You’ll have documentation time every morning and afternoon every day. The entire center shuts down during these times; everyone can lean on each other.
  • Clinical collaboration: We come together in small groups with every therapy discipline represented to review and collaborate on a patient we’re seeing to improve the approach of care.
  • Clinical leadership opportunitiesWe not just have optional opportunities to give back to the field by developing COTAs and new grad OTs, but there is mobility to have the training and support to take on clinical leadership roles managing small teams, single locations, and even regions.
A preview of Inspire Perks:
  • Monthly student loan repayment
  • FREE CEUs internally – not just fluff, but tactical insights that our therapists have spoken highly of
  • Comprehensive Health, Dental, Vision, and Short-term Disability insurance
  • 4 weeks of paid time off between holidays and PTO
  • We're Just Good People: Simply said - we are not jerks; Plus, we like to sprinkle in unexpected fun along the way!
Work schedule: full-time and flexible part-time opportunities available:
  • For full-time, a balanced 4-day work week. 10-hour shifts: Mon – Thurs or Tues – Fri (7AM – 6PM with 1 hour lunch)
  • For part-time flexible schedule options: You can commit anywhere between 16 – 26hrs/week through our morning shift, afternoon shift, or 10-hr shift; a 6-month commitment is required before you change your shift schedule again.

Voluntary Self-Identification of Disability
Voluntary Self-Identification of Disability Form CC-305
OMB Control Number 1250-0005
Expires 04/30/2026
Why are you being asked to complete this form?

We are a federal contractor or subcontractor. The law requires us to provide equal employment opportunity to qualified people with disabilities. We have a goal of having at least 7% of our workers as people with disabilities. The law says we must measure our progress towards this goal. To do this, we must ask applicants and employees if they have a disability or have ever had one. People can become disabled, so we need to ask this question at least every five years.

Completing this form is voluntary, and we hope that you will choose to do so. Your answer is confidential. No one who makes hiring decisions will see it. Your decision to complete the form and your answer will not harm you in any way. If you want to learn more about the law or this form, visit the U.S. Department of Labor’s Office of Federal Contract Compliance Programs (OFCCP) website at www.dol.gov/ofccp.
